Service plans are built collaboratively with each resident and their care team and updated as needs and goals change.
Staff support residents with medication routines and communicate with prescribing providers. We do not diagnose or prescribe.
Coaching in cooking, hygiene, laundry, cleaning, budgeting, transportation, and other everyday skills.
Practical work on coping strategies, emotional regulation, communication, and structure — woven into daily life.
Early identification of stressors, safety planning, and trauma-informed de-escalation to support stability.
Ongoing coordination with outpatient providers, referring hospitals, case managers, coordinated care organizations, and community teams.
Support attending medical, behavioral health, and community appointments in line with each resident's plan.
Guided opportunities to participate in meaningful activities, roles, and relationships as readiness allows.
When appropriate and consented to, we work with families, guardians, and natural supports as partners in care.
Discharge planning is part of care from the beginning — coordinating housing, providers, and supports for what comes next.
