Before admission

Discharge planners share documentation, medications, and any safety considerations. Northvale confirms clinical fit and coordinates timing.

Day of transfer

Medication continuity, transportation, and orientation are planned in advance so the first day feels organized rather than rushed.

After admission

The care team stays in contact with outpatient providers, case managers, and CCOs to support ongoing coordination.
