How to Make a Professional Referral

A well-prepared referral moves faster and helps the clinical review team make an informed decision. This guide summarizes what is typically needed.

Step 1 — Initial contact

Reach out to Northvale admissions by phone or through the referral form. Share basic information about the individual, the current setting, and the reason for the referral.

Step 2 — Send documentation

  • Current clinical assessment or psychiatric evaluation
  • Medication list with prescribing providers
  • Recent behavioral history and any safety information
  • Physical health summary
  • Guardian or conservator paperwork (if applicable)
  • Funding or payer information

Step 3 — Clinical review

Northvale's clinical team reviews the referral for appropriateness and communicates back with the referring team about next steps.

Step 4 — Coordination and admission

If the program is a fit and capacity is available, the admissions team coordinates timing, transportation, and orientation.
